Working Together
The Revitalize Collaborative Provider Network brings trusted professionals alongside your medical care — nutrition, fitness and lifestyle, sexual health and psychotherapy, and pelvic-floor physiotherapy.
Some providers work inside defined Revitalize services. Others are trusted practices we send you to. The difference matters, because it changes who books you, who bills you, and who holds your record.
Integrated Partner
Integrated Partners participate directly in defined Revitalize services. They may deliver an included Allied Health Starter Pathway service, coordinate through Revitalize, review relevant information about your care, and — with your consent — send a concise, clinically relevant update back to the Revitalize team.
Once the included Revitalize service is complete, you may continue directly with that provider if you want to.
Referral Partner
Referral Partners are trusted outside providers. Their care is booked directly with the provider, billed directly by the provider, and governed by that provider’s own practice.
Where the referral forms part of your Revitalize care plan, we ask — with your consent — for a concise, clinically relevant update so your care stays joined up.
When we send you to a Collaborative Provider as part of your care plan, the goal is not a disconnected referral. With your consent, we ask for a concise summary covering, where appropriate: the assessment completed, the broad issue identified, major findings, professional recommendations, whether ongoing care is recommended, anything clinically relevant for your Nurse Practitioner or Shannon, and whether Revitalize follow-up is suggested.
Psychotherapy is different. Only information relevant to coordinating your broader care is shared, with your consent. Detailed therapy-session content is not required and is not requested.
Better health is rarely about changing everything at once. Every Thrive term includes one Allied Health Starter Pathway, so you can focus on the additional area that matters most right now.
The structure differs by discipline because professional appointment length and pricing differ. Your clinical team may suggest which type of support appears most useful, but you remain part of the decision — based on clinical relevance, your goals, your preferences, the provider’s approach and availability.
It refreshes with every new Thrive term. You might focus on nutrition during one term, fitness and lifestyle during another, and sexual-health support later if that becomes more relevant. You may choose the same discipline again, a different one, or a different provider. We do not expect you to fix everything at once.
This is not a credit, a gift card or a dollar balance. There is no leftover value, and it cannot be refunded, transferred or exchanged.
Pelvic-floor physiotherapy sits outside the Allied Health Starter Pathway. It is a separate clinical referral and integration pathway, used when an in-person pelvic-floor assessment is clinically important — for example with early ejaculation, a weak or overactive pelvic floor, impaired relaxation or poor coordination, urinary or bowel symptoms, pelvic pain, assessment before prostate surgery, or continence and sexual recovery afterwards.
A physiotherapist may assess strength, endurance, resting tone, relaxation, coordination and pain, alongside relevant urinary or bowel findings — and whether strengthening is appropriate at all. You may then continue with the physiotherapist, with Revitalize, or with both, depending on what the assessment finds.
An assessment does not mean EMSELLA® is automatically appropriate. Whether it is a reasonable adjunct is a clinical judgement, made for you specifically.
Because many of our Collaborative Providers also operate their own practices, appointment availability is coordinated through our Patient Care Coordinator rather than through a shared public calendar.
Tell us who you would like to work with, or tell us what you are hoping to address and we will suggest who fits. Each provider profile sets out their credentials, whether they are an Integrated Partner or a Referral Partner, whether they work virtually or in person, where they are based, their areas of focus and treatment philosophy, who they tend to be a good fit for, any Revitalize starter service available with them, the ongoing services they offer directly, and their own ongoing pricing where they choose to publish it.
