The Procedure

How the O-Shot® works

The O-Shot® was developed by Dr. Charles Runels and is a trademarked protocol performed only by certified providers. Shannon Lapointe is a Canadian Clinical Instructor for the full Runels procedure suite — she trains other providers across North America in this procedure.

Blood Draw

A small amount of your own blood is drawn — similar to a routine lab test.

PRP Preparation

Your blood is processed in a centrifuge using the Health Canada-approved MD Biologix system to concentrate the platelets and growth factors.

Numbing

A topical anaesthetic cream is applied to the treatment area and allowed to work for a full 25 minutes. An additional injectable block is used if needed. Most patients are surprised by how comfortable the procedure is.

Injection

The concentrated PRP is injected into the clitoral tissue and anterior vaginal wall — the areas responsible for sensation and bladder control. The injection itself takes only a few minutes.

Published Research — Verified Source

When the O-Shot® PRP is combined with the Clitoxin™ neuromodulator in the same appointment, published research by Runels and Runnels (Journal of Women's Health Care, 2024) showed the Female Sexual Function Index total score improved by an average of 12 points — enough to move the average woman presenting with sexual dysfunction to the average score seen in women who describe their sex lives as satisfying. Neither of the two FDA-approved drugs for female sexual dysfunction (flibanserin or bremelanotide) achieves this magnitude of improvement. This data is from peer-reviewed published research. We do not auto-generate statistics.

Research — What the Evidence Actually Shows The O-Shot® protocol is built on PRP's well-documented mechanism: platelet growth factors (PDGF, TGF-β, VEGF, EGF) stimulate angiogenesis, neurogenesis, and tissue regeneration at the injection site. Applied to clitoral and anterior vaginal wall tissue, these growth factors improve blood flow, nerve sensitivity, and tissue health in the areas responsible for arousal, orgasm, and urinary control. Published research specific to PRP for female sexual function has shown improvements in Female Sexual Function Index (FSFI) scores for arousal, lubrication, orgasm, and satisfaction. A pilot study by Runels (Journal of Women's Health Care, 2014) was the first to document the O-Shot® protocol specifically. When the O-Shot® PRP is combined with the Clitoxin™ neuromodulator in the same appointment, published research by Runels and Runnels (Journal of Women's Health Care, 2024) showed an average FSFI total score improvement of 12 points — enough to move the average woman presenting with sexual dysfunction to the average score seen in women who report satisfying sex lives. For comparison, flibanserin (FDA-approved) improves FSFI by an average of 5.3 points; bremelanotide (FDA-approved) by an average of 3.6 points. Neither reaches the improvement threshold needed to move a woman from dysfunction to normal function. The O-Shot® protocol — injecting both clitoral tissue and anterior vaginal wall — is the specific, standardized version of PRP for female sexual function. Why the O-Shot® and Clitoxin™ Are Frequently Combined

PRP regenerates tissue from within — improving the health and responsiveness of the treated tissue over weeks and months. The Clitoxin™ neuromodulator addresses the autonomic nervous system pathway to the arousal center — raising the baseline arousal set point. They work through different mechanisms on the same outcome. The combination in a single appointment produced the strongest results in Runels' published research. Shannon will discuss whether this combined approach is appropriate for your situation at your assessment.

Breast Cancer Survivors

For women who cannot use estrogen

Women treated for breast cancer often cannot use estrogen — meaning vaginal atrophy, dryness, and painful intercourse go unaddressed for years. This is one of the most significant gaps in post-cancer care for women, and it affects quality of life, relationships, and wellbeing profoundly.

The O-Shot® does not involve hormones. It uses your own growth factors to restore tissue health locally — without affecting systemic estrogen levels. Two peer-reviewed studies have specifically examined PRP for genitourinary syndrome in breast cancer survivors:

Published Evidence — Breast Cancer Survivors Hersant et al. (Menopause, 2018) demonstrated significant improvement in vulvovaginal atrophy symptoms in postmenopausal breast cancer survivors following PRP injection, with results lasting up to 12 months. Chen et al. (Obstetrics & Gynecology, 2025) confirmed meaningful improvement in genitourinary syndrome of menopause in breast cancer survivors treated with PRP. Both studies reported no serious adverse events. Shannon regards women who cannot use estrogen following breast cancer treatment as one of the most important and underserved populations she treats. This is not a secondary use of the O-Shot® — for these women, it may be the only viable tissue-level option available.

Results in breast cancer survivors typically last 9–18 months. Shannon will discuss your oncology history at your assessment and confirm appropriateness before proceeding.
