Most ED is a blood flow problem. Research Summary — GAINSwave® / LiSWT for ED Low-intensity shockwave therapy (LiSWT) has the strongest non-surgical evidence base of any regenerative approach to erectile dysfunction. The mechanism is well-established: acoustic waves cause controlled microtrauma in penile tissue, triggering the release of angiogenic growth factors (VEGF, eNOS, PCNA) that stimulate new blood vessel formation. In the penis, where vascular insufficiency is the primary driver of age-related ED, this directly addresses the underlying cause rather than managing symptoms. A 2024 systematic review and meta-analysis presented at the Sexual Medicine Society of North America found LiSWT was the most efficacious single regenerative treatment for ED, with significant improvement in IIEF scores versus sham control. Long-term follow-up from a randomized sham-controlled trial (University of Virginia, 2024) demonstrated statistically significant improvements in erectile function at both one and two years post-treatment — among the strongest long-term evidence of any non-surgical ED intervention. A separate systematic review (Fode et al.) covering 14 controlled trials found consistent improvement across patient populations including those who had failed PDE5 inhibitors (Viagra/Cialis).
