Revitalize Your Health Inc. ("Revitalize Your Health," "we," "us," or "our") operates a private medical clinic in Niagara Falls, Ontario, and provides virtual care to patients across Ontario. We are committed to protecting the privacy and confidentiality of your personal information and personal health information. This policy explains what we collect, how we use, store, and protect it, when we disclose it, and the choices and rights available to you.

As a health-care provider, we act as a Health Information Custodian under Ontario's Personal Health Information Protection Act, 2004 ("PHIPA"). Our handling of your personal health information is governed by PHIPA and by other applicable privacy law.

Information we collect

Depending on how you interact with us, we may collect:

  • Contact details — your name, email address, phone number, and city/region.
  • Assessment responses — the answers you provide in our online self-assessment tools (for example, the FSFI, PFDI-20, IIEF, SHIM, and EHS), which you may choose to have emailed to you and to our clinic.
  • Personal health information — the health history, symptoms, concerns, treatment records, and other clinical details you share with us in the course of care.
  • Payment information — when you purchase a product or service, payment is processed by Stripe, our third-party payment processor. We do not collect or store your full credit- or debit-card number. Stripe processes your card details under its own privacy and security terms; we receive only limited order and confirmation information (such as items purchased, your name, shipping address, and confirmation that payment succeeded).
  • Booking and communication information — appointment details and the content of messages you send us through our website forms, email, phone, or text.
  • Website usage data — standard analytics information such as pages visited, device and browser type, and general location, collected through cookies and similar technologies (see "Analytics and cookies" below).

How we collect it

We collect information directly from you — through our website contact, newsletter and callback forms, our online booking system, our self-assessment tools, our online shop checkout, and during in-person or virtual appointments — and, where you have consented or where permitted by law, from other health-care providers involved in your care.

How we use your information

  • To provide, coordinate, and follow up on your medical care and treatment.
  • To return your assessment results to you and, with your consent, to prepare for your care.
  • To respond to your questions and schedule appointments.
  • To process and fulfil product orders and arrange shipping.
  • To administer our clinic, including billing, records, and quality improvement.
  • To send you information you have requested, such as newsletters (which you may unsubscribe from at any time).
  • To meet our legal, regulatory, and professional obligations.

Consent

We collect, use, and disclose your personal health information with your consent, or as otherwise permitted or required by PHIPA and other applicable law. In most cases, your consent to the use of your information for the purpose of providing you care is implied when you seek treatment from us. Where we email your assessment results, we ask for your express consent first. You may withdraw or withhold consent, subject to legal and clinical limitations, by contacting us — though doing so may affect our ability to provide certain services.

How we store and protect your information

We maintain physical, technical, and administrative safeguards designed to protect your information against loss, theft, and unauthorized access, use, or disclosure. Access to personal health information is limited to those who need it to provide or support your care. Information handled by our service providers (for example, our booking system, website-form provider, email provider, and payment processor) is stored on their secured systems under agreements or terms requiring them to safeguard it. Some providers may store or process data outside Ontario or Canada; where that occurs, the information remains subject to safeguards and to applicable law. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

When we disclose information

We do not sell your information. We may disclose your personal information and, where applicable, personal health information:

  • To other health-care providers involved in your care, with your consent.
  • To pharmacy partners and laboratories as necessary to fill prescriptions or complete testing you have requested.
  • To service providers who process information on our behalf — including Stripe (payments), our online booking provider (Jane), and our website and email providers — under agreements or terms requiring them to safeguard it and use it only to deliver their service.
  • Where required or authorized by law, or to a regulatory body exercising its lawful authority.

Analytics and cookies

Our website uses Google Analytics to understand how visitors use the site so we can improve it. This tool may set cookies and collect de-identified usage data such as pages viewed and general location. It is not used to collect your personal health information. You can control cookies through your browser settings and can opt out of Google Analytics using Google's browser add-on.

Retention

We retain personal health information for as long as necessary to provide care and to meet our legal, regulatory, and professional record-keeping obligations, after which it is securely destroyed or de-identified. Order and payment records are retained as required for tax and business purposes.

Your rights

Subject to PHIPA and other applicable law, you have the right to:

  • Access the personal information and personal health information we hold about you.
  • Request a correction to information you believe is inaccurate or incomplete.
  • Ask questions or raise concerns about how we handle your information.
  • Withdraw consent, subject to legal and clinical limitations.
