I get this question fairly often, sometimes from patients who did their EMSELLA series somewhere else entirely: “It worked great at first, so why did things start slipping back a few months later?”
I understand the frustration completely, and I want to give you an honest answer instead of just selling you another package and hoping that fixes it.
EMSELLA is genuinely excellent at what it actually does. It delivers thousands of supramaximal pelvic floor contractions in a single session, far more than anyone could do on their own, even with perfect technique. That’s real, measurable strengthening, and for a lot of women and men, it makes a real difference fast. But here’s the part that often gets left out of the conversation entirely: a strong pelvic floor sitting on top of unaddressed hormonal changes, or unaddressed nerve or vascular issues, is still working against a headwind. The chair strengthens a muscle. It doesn’t rebalance your hormones, and it doesn’t fix a nerve or circulation problem underneath it.
Why isolation treatment tends to fade
This is exactly why I don’t believe in treating any of this piece by piece. If declining estrogen is thinning tissue and weakening support structures at the same time you’re doing chair sessions, the chair is fighting a losing battle against a process that keeps actively working against it in the background. You’ll see real results initially, because the muscle genuinely did get stronger. But if the underlying hormonal or tissue-level issue never gets addressed, gravity, aging, and biology eventually catch back up.
It’s a bit like doing a great strength program while eating in a way that actively works against your goals. The training genuinely helps. It’s just not the whole story, and pretending it is sets people up to be disappointed later and blame the treatment instead of the actual gap in the plan.
What actually holds
The patients who keep their results long term are almost always the ones treating this as a full picture: pelvic floor strengthening, hormone optimization where it’s relevant, and ongoing maintenance, not a single one-and-done series treated as a permanent fix.
If your results faded, that doesn’t mean EMSELLA doesn’t work. It usually means it was only ever addressing one piece of a bigger puzzle.
