Thrive by Revitalize is one integrated virtual-care program covering hormone concerns, PCOS, weight management and metabolic health. These are not separate services and you do not choose between them — they are connected, so they are managed together where clinically appropriate. There is no separate weight-loss program.
Your first step is a Complimentary Thrive by Revitalize Virtual Care Information Appointment, with our Patient Care Coordinator.
It is a 20-minute phone appointment, not a clinical appointment. You can book it simply to ask questions. Booking it does not commit you to a Thrive program, and if you decide you are ready to go ahead, your Patient Care Coordinator can help you with the next steps in the same appointment. She explains how the program works, what the assessment involves and what everything costs, and answers your questions. She does not diagnose, give medical advice, choose treatment or prescribe — that only ever comes from a practitioner.
There is no charge and no obligation. You must be located in Ontario to receive virtual care.
The Comprehensive Nurse Practitioner Medical Assessment is $399. It happens in two parts with your Nurse Practitioner, separated by blood work: you meet, you go for labs, then you meet again with your results in hand.
These are two separate professional appointments and each is billed when it happens. Together they come to $399. The second appointment sometimes happens a few weeks later, and occasionally a patient decides not to go ahead with it — you are billed for the care you actually receive.
The fee pays for the assessment itself. It does not enrol you in Thrive, and it does not include medication, supplements or laboratory charges. It does not guarantee treatment, prescribing or acceptance into a program.
A valid Ontario health card is requested when you book and must be provided no later than your first Nurse Practitioner appointment. You will be asked to show your card at your first Nurse Practitioner appointment so we can verify your identity, and we record that the verification took place. We store your health card number in your clinical record. We do not keep a photograph or copy of your card. Without it, clinical care cannot proceed and the applicable appointment charge remains due.
3 · Already in care somewhere else?
You are welcome here. Every new Thrive patient completes the same Comprehensive Nurse Practitioner Medical Assessment — $399, including patients whose treatment is currently prescribed and monitored by another provider. Bring any recent results you have; your Nurse Practitioner will take them into account. Revitalize does not assess or take over care for anyone using hormone treatment obtained informally.
This is the part people most often misunderstand, so we say it plainly. Paying for your assessment does not sign you up for anything.
5 · There is no pressure to continue. You may complete the assessment, receive your findings and recommendations, and decide that is all you want. That is a perfectly normal outcome and nobody will chase you.
6 · Revitalize does not provide prescribing outside an active Thrive program. Before a program can begin, all of the following must happen:
Thrive is paid in full when you enrol, for the six- or twelve-month term you choose. We do not offer monthly instalments; if you would like to spread the cost, Beautifi is an independent third-party lender and that financing is an agreement between you and Beautifi, not a Revitalize payment plan. Program fees are generally non-refundable. If your Nurse Practitioner determines that you have become medically unable to continue, we will look at your circumstances individually.
Program payment is made in full or through approved Beautifi financing, and active patients maintain an approved payment method or security arrangement on file.
Four levels. All of them deliver the same standard of medical care — the same Nurse Practitioner care, the same comprehensive assessment, the same individualized laboratory investigations, the same treatment options. What changes is how much support and access you have around that care.

Thrive Starter Care — 6 months — $1,000
Where most people begin. Six months is long enough to establish treatment and find out what you actually need.
Thrive Optimize — 12 months — $2,000
A full year of regular progress work alongside your medical care, with both dietitian and fitness assessments included.
Thrive Signature — 12 months — $2,800
Our most structured level — a progress appointment every month, three times as many as Thrive Optimize. Same medical care, closer support around it.
Thrive Lasting Care — 12 months, renewable — $1,500
For when your treatment is settled and working. Dietitian and fitness support are not included at this level, but both remain available and we encourage them.
Your program level never changes your medicine. It does not change how comprehensive your assessment is, which laboratory tests are ordered, which treatments you are eligible for, or what your Nurse Practitioner decides. There are no “basic” and “advanced” laboratory panels here — your investigations are individualized to your clinical need.
This is where your plan actually gets worked. It is a structured progress review with your Patient Care Coordinator — not a medical appointment, and not a check-in for its own sake.

Your Patient Care Coordinator does not diagnose, give medical advice, select treatment or prescribe. Anything clinical is routed to your Nurse Practitioner.
Ordinary clinic coordination is never withheld. Help with appointments, forms, bloodwork logistics, pharmacy questions and getting information to the right person is part of being a Revitalize patient at every level. What the higher levels add is planned, proactive progress work — not permission to contact us.
Local patients may receive complimentary in-clinic InBody® 380 assessments as a program benefit where appropriate. This is a local convenience, not a higher-level benefit, and not receiving it does not mean lesser care. A home InBody® scale is available separately if you would like consistent tracking at home.
Your dietitian and fitness support are described in full on their own pages: Registered Dietitian care and Fitness & Lifestyle support. Both can also be used without a Thrive program.
Your Nurse Practitioner appointments are 50 minutes of a prescriber's time and cannot be refilled at short notice, so we ask for as much warning as you can give:
These fees are in addition to your program fee. A missed appointment does not replace your medical follow-up — you still need to be seen, and the fee is payable before your replacement appointment. This is not about the money; it is because the follow-up is part of how your treatment stays safe.
Near the end of your term we will review how your care is going. Your Nurse Practitioner considers what monitoring you clinically need, and we then talk about the level of support you would like around it.
You may stay where you are, or move up if you would like more support. Lasting Care is the exception: it is not a starting point, and it becomes available once you have completed the applicable period of prior Thrive care and your Nurse Practitioner has determined that your treatment is settled. Ask us and we will tell you exactly where you stand.
Nobody is required to move up a level. Staying where you are is a perfectly good decision, and many people do. If your circumstances change, your Nurse Practitioner may recommend more frequent follow-up or a different level of support again — that is normal, and it is not a setback.
You complete the term you enrolled in, and choose again at renewal. That way your Nurse Practitioner can look at the whole clinical picture before anything changes.
You choose a program first. Optional add-ons come afterwards, and only if you want them. Add-ons may include additional Registered Dietitian appointments, additional Nurse Practitioner appointments, additional home blood-draw collections, and a home InBody® scale.
9 · The home InBody® scale is optional and is not included in any Thrive program. It is available as an add-on with any Thrive program, and we may recommend one where consistent at-home body-composition tracking would help.

Your program fee covers your care — the appointments, the coordination, the monitoring and the support between visits. It never includes the cost of your medication.
Medication, supplements and laboratory charges are separate. Prescriptions are yours to fill at whatever pharmacy you like, and lab fees are billed directly by the laboratory.
Your program is delivered in three-month medical-care periods. Each period includes a Nurse Practitioner medical review and the monitoring, prescribing decisions and coordination that go with it. A six-month program has two of these periods; a twelve-month program has four.
Your Nurse Practitioner carries clinical responsibility. She assesses you, makes the clinical decisions, gives the recommendations, and prescribes where that is clinically appropriate and within her scope.
Your Patient Care Coordinator is a non-clinical role. She explains how the program works, supports your onboarding, reviews general program and pricing information, and keeps your care organised. She does not diagnose, give medical advice, select treatment or prescribe.
Our Registered Dietitian provides the nutrition side of your program.
Your records are held in Jane, our booking and records platform, and your care is delivered by secure video or phone. We communicate with you through Jane, by telephone and by other approved methods you agree to.
Virtual care has real limits. Your practitioner cannot physically examine you, and some concerns need to be seen in person. If that is the case, we will tell you and help you find the right place.
Revitalize Your Health does not provide emergency care, and Thrive is not an urgent-care or after-hours service. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
